Role description
Position Information
The Department of Education supports secondary students with disability, and their families, to successfully transition from school to meaningful post-school pathways through a student-centred planning approach. This process involves strong collaboration between students, families, schools, School Services teams, and a range of community-based education, training, employment and specialist service providers.
The Support Teacher Transition (STT) is a specialist role that works alongside schools to facilitate personalised transition planning for secondary students with disability.
In supporting students, families, teachers and allied support staff, the Support Teacher Transition may:
